THE FIRST SLIDE OF YOUR PRESENTATION MUST LIST YOUR FINANCIAL RELATIONSHIPS.  FULL DISCLOSURE IS REQUIRED FOR ALL SESSIONS.

• Download the template below
• The template includes 4 sample slides
• You do not need to use all 4 slides, only 1slides is needed
• If you speak more than 1 time, use the same FULL DISCLOSURE slide for all of your sessions
• All speakers must have a disclosure slide even if there is nothing to disclose
